If you are filling out new patient paperwork, it is your responsibility to notify the clinic when this is completed. We have no way of being notified this is done. To prevent delays in scheduling, please call as soon as you have filled the paperwork out in it's entirety. Incomplete paperwork will delay scheduling, as you will be asked to redo it.
